STUDENT GUARDIAN (SUBCLASS 580) VISA CHECKLISTNote that the Assessment Level of the student who nominates the applicant as their guardian affects the applicant’s eligibility for this visa. |

Form 157G – Application for a Student Guardian visa (fully completed, signed and dated by the applicant). |
|
|
Two passport size photographs (4cm x 6cm). Photographs must not be more than 6 months old. |
|

Form 157N – Nomination of a student guardian (fully completed, signed and dated) |

Fully completed Details of Relatives form. |
|
|
Certified copy of biodata page of passport for applicant and any family members included in application (passports must be valid for at least a further 6 months). Note: Original passports should be provided after applicants have undertaken medical examinations when requested by our office. |

|
Original Curriculum Vitae (CV), including details of education background and employment history, certified by the local authority (must not be more than 6 months old). |
|
|
Certified copy of all pages of current Household Registration Book. |
|
|
Certified copy of Birth Certificate. |
|
|
Evidence of relationship status:
|
|
|
Evidence of relationship with the nominated student, in the form of certified copies of Birth Certificates. |
|
|
If you have family members remaining in Vietnam, evidence of child care arrangements for any children under 18 years of age. |
|
|
A consent letter signed by both parents (or the other parent, if you are a parent of the student). |
|
|
If currently employed, evidence of that employment (such as employment contract, letter from employer). |
|

Health and character documentationMedical examinations cannot be undertaken before an application is lodged. After application lodgement, our office will contact the applicant with instructions about undertaking medical examinations. |

Evidence of valid Overseas Visitors Health Cover (OVHC) for applicant (and any family members included in the application) for at least the first 12 months of the period of stay. |
|
|
Original police certificate from each country where the applicant (since turning 16 years of age) has resided for a total of 12 months or more in the last 10 years. If they need one from Australia, they must obtain an Australian National Police Check (NPC) from the Australian Federal Police (AFP). Only a “Complete Disclosure” NPC is acceptable. When completing the Australian NPC application form, Code 33 must be used at Question 1. See: http://www.immi.gov.au/allforms/character-requirements/ |

If family members are included in the applicationNote: A Student Guardian visa applicant cannot bring other family members to Australia with them, unless they have dependent child(ren) who are under 6 years of age and compassionate and compelling circumstances exist. |

Certified copy(s) of Birth Certificate(s) for dependent child(ren). |

If the family member is the primary applicant’s child and the other parent is not applying to travel, consent from the other parent for the child to travel to Australia with signature witnessed by the local authorities. |

|
Evidence of sufficient funds to support dependent child(ren)’s stay in Australia. |
|
|
Evidence of compassionate and compelling circumstances for taking dependent child(ren) to Australia. |
