STUDENT GUARDIAN VISA (SUBCLASS 580) CHECKLIST - The assessment level of the student who nominates you as their guardian affects your eligibility for this visa.  Please be aware that some visa subclasses are subject to Pre-Visa Assessment (PVA)                                                                                                                    

Requirements - Primary Applicant

Client Use               
A fully completed and signed form 157G.  
Four passport sized photographs (4cm x 6cm).  Photographs must not be more than 6 months old.  
Original signed passport (must be valid for at least the next 6 monhts).  
A fully completed and signed form 157N.  
Totally completed details of relatives form, see: http://www.vietnam.embassy.gov.au/hnoi/files/relativesform.pdf  
Original Curriculum Vitae (CV), including all details - education background and employment history, certified by the local authority (must not be more than 6 months old).  
Certified copy of all pages of current Household Registration Book (HHR).  
Certified copy of Birth Certificate (BC).  
If married, certified copy of Marriage Certificate (MC).  
If have dependents, evidence of child care arrangements for dependent children remaining in Vietnam.  
Evidence of relationship with the nominated student in form of certified copies of BC.  
A consent letter signed by both parents (or one if you are a parent of the student).  
If currently employed, evidence of that employment.  
Original Police record certificate.  
Financial Documentation - see visa requirements of the visa the student who nominates you as their guardian is applying for  
Original signed passport (must be valid for at least the next 6 months).  
Confirmation that Medicals have been completed with a panel doctor.  
Evidence of valid Overseas Visitors Health Cover (OVHC) - paid for the first 12 months of stay in Australia.
