MEDICAL TREATMENT VISAS

This information is provided to assist applicants in completing the visa application process in Vietnam. Applicants must also read the information regarding visiting Australia on the Department of Immigration and Citizenship (DIAC) main website.

This information is intended for applicants wishing to visit Australia for medical treatment or medical consultations.

This information must be read in conjunction with the Vietnam General Information sheet which provides details on lodging a visa application in Vietnam.

DOCUMENTS REQUIRED: The following is a list of documents that should be submitted in support of a Medical Treatment visa application. This list specifies the minimum documentation only and is not exhaustive. It is open to applicants to submit any documentation they wish which they believe will assist in the assessment of their application.

Information relating to the applicant
□ Form 48ME - Application to visit Australia for medical treatment

□ Two passport size photographs (4cm x 6cm) for each applicant aged under 70 years and Four photographs for each applicant aged 70 years or over. Photographs must not be more than 6 months old
□ Signed Passport. Expired and unsigned passports are not acceptable
□ Details of Relatives Form

□ Certified copy of all pages of current Household Registration Book
□ Certified copy of Birth Certificate
□ Original Curriculum Vitae (CV) certified by the local authority. Applicants employed by state-owned agencies/enterprises should have their CV verified by the employer. CV must not be more than 6 months old
□ Certified copy of Marriage Certificate, if married
□ Evidence of sufficient funds to cover all medical and living expenses for applicant and family members for the duration of the proposed stay in Australia (e.g. evidence of your income in Vietnam, evidence of bank savings etc). Certified copies of original bank books / documents to be provided
□ Evidence of house or land ownership or other significant assets
□ If working - full details of your work including your title, work address and work phone number and a letter from your employer confirming your leave
□ If a child under 18 years of age is travelling, a written authorisation letter from both parents of the child allowing him/her to travel. The signatures of the parents on the letter must be certified by the local authority


Information relating to medical treatment
□ Name and address of the referral source (e.g. the referring doctor or hospital in the applicant’s home country)
□ Name and address of the relevant medical practitioner or medical facility in Australia (i.e. the hospital or clinic in Australia to which the applicant has been referred for treatment)
□ Written confirmation from the medical practitioner and/or medical establishment that
- they agree to treat the patient
- the nature and estimated duration of the treatment
- proposed dates of treatment and/or consultation
- a proposed date of admission to the relevant medical facility (if applicable)

Information relating to private hospital/clinic or private medical practitioner treatment
If the treatment is to be provided by a private hospital/clinic or by a private medical practitioner (including a private medical practitioner in a public hospital), the applicant should provide:
□ written confirmation from the relevant hospital, clinic or medical practitioner that
- they undertake to treat the applicant; and
- they are satisfied as to the arrangements for the payment of all costs related to the treatment (including fees for consultation, treatment and hospital accommodation); or
- the relevant hospital, medical practitioner and/or other health care providers have agreed to give their services free

Information relating to public hospital/clinic treatment
If the treatment is to be provided by a public hospital/clinic in Australia, the applicant should provide:
□ Written confirmation from the relevant Australian State/Territory health authority, that
- the authority undertakes to treat the applicant in that public hospital; and
- no Australian will be disadvantaged
- the authority is satisfied as to the arrangements for the payment of all costs related to the treatment (including fees for consultation, treatment and hospital accommodation)
